How do I make a claim?

In the event you become aware of a claim or a specific act or omission which could result in a claim for which coverage may be provided in the policy, please immediately send a written notice of such an incident to:

Your written notice must include the following information:

  • Your name, address and business telephone number

  • A copy of your policy’s Declarations Page

  • The specific act, error, omission or personal injury alleged or which may be alleged

  • The date or period of duration of the professional services

  • A brief description of the issue

  • The injury or damage which has resulted or may result

  • The circumstance by which you first became aware of such act, error, omission or personal injury

  • Please send us copies of all letters and legal papers you have received.
  • It is important that you do not communicate with any adverse party or their representative about the claim or potential claim.  You must not admit liability nor should you attempt to handle this matter on your own.  These actions may jeopardize your coverage. 
    Please refer to your policy for a complete definition of a “Claim” and a description of our respective obligations.

    A representative from our Claims Department will contact you after your notice is received and initially reviewed.  You will be given a claim number.  We will discuss with you the further handling of this matter.

    You may contact the AFPD Claims Department by telephone prior to submitting the required written documentation; however, such telephone contact shall not constitute notice of a claim or potential claim as required by the policy.

    I am an independent broker with a potential insured, how do I access the Preferred Program?

    The Preferred Program is accessed through a select group of appointed agents.  Agents are appointed in each of the states where the Program is available.  For an agent in your area please contract Irwin Giles at ifgiles@afpd.net or at (800) 870-7750.

    I am an independent broker with a potential insured, how do I access the Alternative Program?

    The Alternative Program is accessed through any excess and surplus line agent possessing a Surplus Lines license in the state where the prospective insured is located.  For assistance in locating a surplus line agent in your area please contract Irwin Giles at ifgiles@afpd.net or at (800) 870-7750.
